If you are interested in becoming a patient of ours, please read the information below to get started:
- Our standard practice is we require a new patient referral from one of your existing health care providers. Typically, the referral comes from your primary care physician. If you need assistance with getting a referral to our office, please let us know.
- Our office accepts many medical insurance plans including, but not limited to: Aetna, BCBS, BCN, CIGNA, McLaren, Medicare, UM Health Plan (formerly PHP), Priority Health, Sparrow Care Network, Tricare & Most Commercial Insurances.
If you have already scheduled a new patient appointment with us, you will receive a new patient information packet from us in the mail. However, if you misplace any of the items in the information packet you can find many of these same items below.

- Records Release (send to your doctor so they will release records to us)
